i still not so sure about how to write proper exercise prescription in my assessment..
such as

2.4.1:Heat therapy

- Sitting ; Short Wave Diathermy ; Right shoulder ; 20 minutes.

2.4.2:Joint Mobilization with movement

- Supine lying; Oscillation technique grade 1; Right shoulder superior glide,inferior glide, anterior glide & posterior glide ; 30 x ; slowly
- Side lying on left side ; Oscillation technique grade 1; right scapulae glide, inferior glide,lateral glide & medial glide ; 30 x ; slowly









2.4.3: Mobilization and stretching exercise

- Standing ; right shoulder finger ladder exercise flexion & abduction ; 50 x slowly
- Sitting ; right shoulder adduction & abduction ( hand hold arm wheel ) ; 50 x alternately ; slowly
- Sitting ; right & left shoulder flexion & extension ( hand hold overhead pulley );
- 50 x alternately ; slowly

2.4.4: Strengthening exercise

- Stride standing; right shoulder flexion, extension, abduction & adduction ( hand hold theraband + yellow colour) ; 20 x ; slowly

2.4.5: Home exercise programme

- Standing ; right shoulder finger ladder exercise flexion & abduction ; 50 x ; slowly
- Prone lying; right shoulder flexion & extension (hand hold bottled fill with water + 500 ml ); 30 x ; slowly
- Stride standing; right shoulder flexion, extension, abduction & adduction ( hand hand hold bottled fill with water + 500 ml ) ; 30 x ; slowly

......

a) Cryocuff at Rt knee, supine lying with Rt. knee elevated, 20 minutes
b) Passive stretching for Rt. hamstring, sup.ly., 3x, sustained 10 sec
c) Strengthening exs for Rt. quad, IRQ, long sitting,pillow, 3 sets, 10 x, hold 5 sec
d) HEP Pamphlet given
e) Advice pt. to do exs as taught and elevated Rt. leg when sleep

......



- MFR at right iliopsoas piriformis until the tautness is reduced, pt. prone ly.
- Neural tension exs. until the tautness is reduced, pt. side lying on left side
- McKenzie back extension in lying within pt. pain tolerance, 10 reps, pt. prone lying
- Lumbar stabilization exs, 3 sets, 10 reps., hold 10 s., pt. supine lying
- Active stretching for right piriformis, 4 sets, hold 15 s.
- Strengthening exercise for right gluteus maximus and medius, prone and side lying on left side, 3 sets, 5 reps, hold 5 s.
- SWD for lower back, pt. prone lying, 20 minutes, 50 hz, continous mode
- Lateral shift correction to the left and back ext. exs. In standing within pt. pain tolerance, 10 reps.
- HEP do exs. as taught at home and maintain the correct body posture.

Hi nhafifi,

it sounds like you are looking for some shorthand method of writing the treatments. Have you or your university got a copy of the maitland peripheral manipulation book .... it has a great section on how to record the various grades and direction of movement ?

Its quite an expensive book but here is a link if you have not seen it :

What you have written makes sense to me but I wonder if anyone else has any thoughts ?
